Have you tired doing a screen share in Messenger? I can't remember if it shares audio also or not.

What might work is Team Viewer, where the client could see my desktop as I work, but I'm not sure he can hear a sequence play on his computer (VOIP) while I am playing it on mine. Simultaneous audio transmission might work over a telephone line, but gets a little cumbersome.
