The video is doing that, or is just the audio?

If just the audio, look at see if the SRC Clip is Mono or Stereo. If there is not a compatible REC track, then it'll create a new one.

If it's doing that with a video track, the first thing to do is make new User Settings and see if the issue continues. If it goes away, then your User Settings are corrupt. Ideally you can delate the bad ones and import a backup set with all your customizations already in place.

That sounds like maybe the clip has stereo or multichannel audio, but your timeline tracks are mono?

If so, you can change the audio channels of the clip by right-clicking the clip in the bin and going to Modify > Multichannel Audio
