In regards to the deck settings... You should try real hard to make sure they match...
You can often get odd errors with cueing and reporting accurate timecode otherwise.
Post the exact deck model, and someone here may know.

I posted a little while ago that I've got a Panasonic AJ3700B deck that never auto configures properly - Avid always thinks it's a Panasonic (something else)

This ALWAYS screws up digitizing. But you can tell right away... it won't even load in to the avid.
However, I've seen assistants do things like turn off the 'TC' button so that their digitizing will work... when instead they should have changed the deck... that would be what is considered a BadThing.
