I got some clarification from the QC folks and apparently the QC room has a regular HD monitor as well as a 4K monitor. According to them they QC on the HD monitor but they did add that they could see the pixel issues on both the HD and the 4K monitor which leads me to believe they are perhaps watching both and maybe they pick up on the error on the 4K monitor then check the HD. Just guessing but these are really minuscule and very hard to see in normal viewing.

Why an earth would you QC HD content on a 4K monitor. Daft.
If the content is HD and shows no issues on a HD monitor and it's for HD broadcast then it's ok.
If the requirement is to deliver 4K that's different.
Its quite possible the 4K upres is introducing the artefact that looks like a dead pixel.
