Well I thought I had fixed the problem with 4 boots in a row with all drives showing up. Then I plugged back in the fibre cable and poof a bunch of internal drives didn't mount. I unplugged the fibre and voila back are all the internal drives on boot up.
Well this now shows that there is some conflict when the fibre is connected to the sanmp system. In the past with a promax fibremax chassis I had to go into bios and tell it not to load the fibre chassis drivers or some such thing so it would not hang on boot up. That was an old meridian symphony on a Compaq EVO-800. Is there some similar way to get the macpro to not load the fibre channel drivers until the internal drives have mounted. And why does this not happen when booting to an exact Carbon Copy Clone on an internal HDD? Talk about herding Cyber Cats

Okay I finally pulled the 850 out to my Voyager dock and reformatted it. I have an older version of Carbon Copy Clone but it has worked fine for all the HDDs. I formated GUID Mac Extended Jounaled with disk utility. Then went to carbon copy clone with the SSD still in the dock. I first installed a recovery partition with eh CCC disk center. Then I went to clone from my solid HDD. Two times the process started and then stalled. First at just under 4 GB and the second, after a complete re partition and a new recovery partition. After that I repartitioned it and redid the recovery partition in the dock but then reinstalled the SSD into Bay one of the MacPro. Now the same version of CCC and it seems to be flying along just passing 30 GB now for a drive that has about 90 GB total.
Curious why the stalls while in the dock? Should I have been more patient and maybe it would have picked back up. The voyager dock was connect through a 3rd party USB 3 card. I wonder if that would have anything to do with the stalls? I've copied a few things this way without issue in the last 2 weeks and to other rugged drives. Anybody got their ones and zeros hats on have any idea?
Hopefully after all this the SSD will boot and see the other internal drive and the internal raids now. That was the issue I was having before. Booting to the SSD and the other drives didn't always show up whereas the SSD CCC's to the other internal HDD booted with all drives showing up all the time. Hope this fixes that.

I actually had my system drive fail last week and replaced it with this 850 in 2010 Mac Pro.
My process was to restore the system to an older drive I had around through time machine. Then I cloned that older drive to the 850 via USB3. I then installed it into the bay above the DVD drive. I have had no issues churning on it since Saturday. I guess I skirted the issue you had.
I saw the firmware you saw, too and decided to avoid it since the 850 wasn't listed.
Not sure if I've been helpful other than confirming the workaround indeed works.

I can repair the drives with disk Utility and get them to mount. I had solid boots for about 3 days with all drives showing up. Today I installed the Sony XD Cam U2 driver like our other Avid systems have. After that a bunch of the internal HDDs went offline. Disk utility repaired and remounted them. In the case of the first partition of a 4TB Deskstar NAS I had to verify just that partition before mounting it then I could repair that partition as "repair" was greyed out for that partition until I verified and mounted it. All the other HDDs including a 2 drive striped internal raid will just let me repair and mount. Only that first partion on the solo HDD that has 10 partitions requires the verify. Now when I reboot on the SSD the first partition is always unmounted and requires the verify process.
When this is going on if I restart to the carbon copy clone of the SSD all drive mount reliably for 3 reboots. It really seems like something about the Samsung SSD 850 EVO 250GB is wreaking havoc as a startup drive. I'm wondering if it were installed up in the 2nd optical bay if that would have any effect on it's reliability? I am now going to take the SSD and Carbon Copy clone it to a virgin partition on the solo HDD that is mac os extended journaled with the proper GUID stuff and see if that will reliably boot. I seem to recall some people not liking the Samsung SSDs but I don't remember the specifics of their issues.
Anybody have any suggestions/experience with these SSDs?

Your drives are most likely working as Samsung intended. I'm currently upgrading several MacBook Pros for coworkers, and ran into this problem. It will neither format nor install via the internal SATA. The workaround is to connect the drive via external USB. From there you can partition/format/install to the drive. After you're done, you can install it.
However, if you ever plan to erase & install, you'll have to take it out and connect it externally all over again.
From what I gather, Samsung does have Mac specific firmwares for their 830 and 840 series SSDs. Descriptions says it addresses compatibility issues, which we're seeing here. The 850 is brand new, so perhaps they'll release the Mac firmware soon so you don't have to jump through hoops to get it to work.
