Thanks guys, and Wilson for pointing to a gem I did not know yet!
Found some more examples in the mean time. Like Jim, I never had come across the need for filling out a risk-assessment in 25 years of production..

The Google has some suggestions (as you probably know). This from Scotland looks actually useful:

Here in the US, I've never been asked to fill out a Risk Assessment form, but have had to convince location owners/bosses that I was not a complete idiot. And I frequently have to provide proof of holding a $1 million general liability insurance policy. And then sometimes a location fee (and this for simple stuff).

I think I'd rather just fill out a form.
