I had a similar problem - never truly solved - in December with XDCAM-EX footage in Media Composer 7.
It was a three-camera shoot with supposedly identical F3 cameras. Producer logged the footage in Media Composer 5.5. I could not ingest one of the three cards in Media Composer 7.0.3 on Mac OS 10.7.
One of the three cards had a different folder structure. There was a DCIM and PRIVATE folders on it at the top level. I suspected that it was recorded to an SD card in a Sony MEAD SxS adapter. (The BPAV folder MAY have initially been inside of the PRIVATE folder.)
I have Media Composer 4.0 on my laptop for emergencies, and that old version had no trouble AMA-linking and ingesting the XDCAM-EX footage.
