My definition of a 'flash' frame is a single frame edit that could be a single colour, or of an unconnected shot.
Proper 3:2 pulldown will have 'mixed' frames containing fields from two shots either side of an edit, but not 'flash' frames (apart from ones intended for creative reasons.)
Of course, if the QC guy was going "field by field" in the wrong order, normal mixed frames would become flash frames...
