It shouldn't be the FS1 as that's not an i/o device for Avid is it? If you toggle hardware on/off in the timeline do you have the problem?

Thanks for the valuable info. I don't think the error occurs on the same frame, but it does happen during scrubbing. As for external devices, I'm using IOExpress and AJA FS1. Do you have any suggestions for out to determine the culprit?

This particular operation is used when single stepping through the timeline, scrubbing, moving the "blue bar" to a new position, or at the end of a streaming play. An example of potential causes are media file issues, either errors reading the file, corrupt media which causes errors in the decoders, errors communicating with external hardware, etc. If the failures always happen on certain frames, it's likely a media/media file issue.
