You could also import the entire document (all pages) into the Gimp.  They show up as layers.  There's a plugin that allows you to export the layers as separate image files, but even without that you can simply export, turn off the top layer's visibility, repeat until each layer is in a separate image file.

On OS X - Preview app then export to bitmap format.

Photoshop, open each PDF page in turn, save as bitmap format of your choice.
