This happens randomly on ProRes QTs. I believe it's a bad read by the QuickTime AMA plugin. Sometimes you can solve it by dumping the linked file and relinking. Most of the time, your solution is the only way out. The QuickTime AMA plugin needs a lot of help still.

Using either 7.0.3 or .4 - Arri ProRes 444 Log C links via AMA just fine. Try to transcode to DNX 220x and this error message:
"Exception: Failed to get the sample position from the AMA Plug-in"
If I cut the clips into sequence and remove the audio, I can transcode the clips just fine.
We saw this error on multiple projects that worked fine on other systems that had v6.4.2
I officially declare this a bug!
