Correct. Sound Devices PIX240. It recorded the SDI output of the F3. NOT converted from the EX. So technically a much better quality recording by far than the EX on the SxS cards.

Correct. It records an interleaved a/v file in QuickTime format using either ProRes or DNxHD codecs. It will also cross-convert on import, if needed. For example, on one edit job I did, they recorded on a Canon C300 as 23.98 and to a PIX240. The SDI output of the camera added 3:2 pulldown for a 29.97 video stream. But the PIX240 did a reverse pulldown to get back to 23.98 on-the-fly.

Sounds like operator error. You can run the PIX240 manually or have the recording triggered by the camera. The recorder has to be properly armed/set up to be triggered by the camera, as I believe it does this by seeing incoming TC. It may also be possible that clips were erased inadvertently if they were playing back from it as a video assist.
