I think I have it under control... finally.
Demuxed the original and created one long VOB file of the entire film, sans audio. Ripped a copy of the picture to DVD codec just for a reference. Made an AIFF version of the AC3 audio. After going crazy trying to deal with sync in programs like DVD Studio I instead went to FCP, fixed the audio sync, exported audio to AC3, and recombined with the demuxed picture file.
Or that's what I will be finishing this afternoon.
Redoing this in HD soon enough, but need a good quality screener with fixes now.
