[Avid-L2] End sample index exceeds on disk Index Entry Count

A 23 minute program just came in for us to color correct. Offline system was MC on Mac v 5.5.2. Our system is Symphony on XW8600 v5.5.2
Most media is EX3 (XDCam native). Playing off of clients GRaid with a fast eSata connection. Done this before many times. Footage was consolidated to this drive. When cursor is parked, media is there. When playing I get the message Exception: MXF_DIDMapper-End Sample Index exceeds on-disk Index Entry Count.

I mixed down all audio tracks to 2 tracks. I deleted media databases. I chopped down the sequence into smaller bits to look for corruption. Same issues.
