I offer:
As others had already mentioned, go to "Grid Settings: Coordinates"
Tab. Set "Scale Mode" to "4x3 Inside 16x9 Monitor".
"Increments" should default to "Fields" 12 and "Subfields" 4.
"Source Scan Size" should default to "Horiz. Size" 720 Pixels and
"Vert. Size 486 Pixels.
"Source Grid Adjustments should all be set to 0.
Under "Grid Settings: Display" Tab, check "Show Safe Title" and "Show
Safe Action". I also check "Show Axes".
Click on "Apply" and "OK". The grid will now display a 4x3 safe box
within your 16x9 video.

I reply:
I tried this and could not export a QT reference file in this
format. You may have to export a self-contained QT instead, using
custom settings and "Crop to fit" in the export settings window.
