Risk Assessment forms a legal requirement for pretty much anything that happens in a public space in the UK. I believe the point is to show that you've been through the process of thinking about what risks exist and what you can do about them (although they've mostly turned into a boilerplate red-tape exercise as these things do). I get sent copies of them all the time for things I'm working on. They're full of things like:
Risk: heavy lifting
Response: manual handling training
Risk: injuries to feet/ankles
Response: appropriate footwear to be worn at all times
...and other things that are even more obvious, like taping down cables. There's usually a comment about having a first aid kit around, but for a small crew I'd imagine having an actual first aider probably isn't necessary.

Found some more examples in the mean time. Like Jim, I never had come across the need for filling out a risk-assessment in 25 years of production..

Here in the US, I've never been asked to fill out a Risk Assessment form, but have had to convince location owners/bosses that I was not a complete idiot. And I frequently have to provide proof of holding a $1 million general liability insurance policy. And then sometimes a location fee (and this for simple stuff).
I think I'd rather just fill out a form.
