I'm not the expert in this part of the code but looking at the output it seems that the thing we call the SWDecodeCvtr is waiting on it's input. That input would be a thing called the "reader" which is the object that reads from disk. Judging from the info about the timeline and the info from console I'd say you've got a disk read problem.

Drives are Samsung, formatted HFS+ Journaled - I always use "not journaled", but this time the setup of the machine was done by my assistant. Could it be a explanation?
