Along similar lines I've seen where have a .mov open in QT 7 seems to hold an older version in some sort of limbo. If you replace the graphic with the same name and QT 7 is still open with the old one I've seen this sort of cross breading. Usually quitting QT 7 alleviates the issue. I've seen this when I make QT Ref too and drag and change a sequence in avid, redo the QT Ref export and then take the new one into Adobe Media Encoder. AME then processes the old QT Ref. Again quitting QT 7 seems to solve this but as has been pointed out often the issue is so fleeting that it's hard to pin down and once solved it's time to move on without investigation.
I think the renaming with a version number should help and you can always reimport that so all the instances will change. I've even had success setting the bin display to show rendered effects to show the imported graphic and changed that name to reflect the new version number and voila all the clip names in the timeline reflect the new version name of the graphic. Not a necessary step but it helps others play along down the line.

Even doing a new import only reveals the old file. WTF?????
How is this even possible??? Can someone tell me please. This is going to make life really difficult if this is the new norm. Nothing changed that I am aware of between normal and this new bizarre problem.
