Thanks Job and Dom for the suggestions - there were 2 monitors on the
machine and after a bit of investigation I found that if I
disconnected the 2nd monitor the problem went away... in this case the
2nd monitor is a smaller resolution than the primary monitor and I
suspect that is somehow confusing Title Tool about where it needs to
display the UI (preview image and title controls). I'm hoping that by
swappinig the current 2nd monitor with one that is equal resolution to
the other monitor it will be solved.
