Generally clearance comes down to each individual TV station. There is no central authority for clearance. 16x9 SD isn't the norm unless the station specifically wants that to blow it up for HD. Each station has their own guidelines. Make sure all graphics are 4x3 safe and be prepared to deliver both a 4x3 center-cut SD and in some cases an HD version (for stations willing to accept HD).
Acceptable formats again range all over the place, depending on station. Most common is BetaSP, then DigiBeta, some BetaSX and some IMX. A few will accept files. Many get their national spots via an electronic delivery service called FastChannel.
