My first thought was un-rewound tapes with "tail bars", which the
assistant thought was "head bars" and hot-rolled as-is into the rig
without any form of machine control (or pre/post verification
obviously). But that seems silly - even a common ingest ferret would
have caught that before heading down to the hookah bar.
And then I thought hey - I've seen some really fuckered-up "clones" in
my day - so it may have happened at that stage (if that stage actually
existed). The dubber may have just loaded in the tape ASSUMING it was
rewound and thought those were head bars - hit "go" on both decks -
and then walked away... coming back to two stopped / rewound decks,
checking to see if the "head bars" stuck, then on to "cloning" the
next tape.
And then I thought hey - who the hell wants to pay $$ for the original
tapes these days - let alone a "clone"; and so I'm back at square one
wondering how the hell shit like this can still happen in an
incredibly competitive job market.

I once nodded off briefly while punching in the top half of a lead
vocal verse - awakening to the poor singer screaming in a panic.
Thankfully I was punching into a spare track on the tape or else it
would have been gone. Tape erase heads don't generally permit any
"undo" :/
