A possible cause:
On Mac, occasionally if drive mounts go wacky, there's a chance they leave a ghost mount in /Volumes. Then when you remount the drive, it will show up in /Volumes again. If the drive is named MyDrive, the ghost mount will be there named MyDrive and the real mount will be named MyDrive-1. Unmount the drive, delete the ghost mount, and remount the drive.
