OK, I may have found my issue in the configuration guide:
Important note – If installing an Avid DX interface HBA or HP Thunderbolt HBA in slot #5, jumper E3 on the Z840 motherboard must be placed on pins #1 and #2. Failure to do so will result in intermittent / unreliable operation of any hardware connected to either HBA
Missed that.

Subject: [Avid-L2] Avid/BMD DXxIO disconnecting?
I have a brand new install of MC 8.4.2 Win7 HPz840 ordered to the Avid spec. Using the original Nitris DX PCIe card in Slot 6 per the configuration guide.
The new Avid/BMD DNxIO is disconnecting or “disappearing.” I run the BMD Desktop Video Utility and it does not show a device connected. Sometimes a reboot will fix the issue, but closing and opening Media Composer may find that it has disappeared.
Anyone seeing this issue?
