I've had the same thing happen.
Symphony v 6.0.4 MAC OS 8.5
I've forced myself to always remember to check all the box settings, all the enabled and monitored tracks and the destination before hitting the enter key.
Well, sometimes I still forget. Old age strikes again.
That said I wish MC would have a customizable setting to default to the highest video track irregardless of track patching.
