Shot by shot in software and prior to uprez. You can't fix this once the master is progressive. The artifacts are baked in.
It may require more than one tool if you are picky. Easier if you are not too picky.
Deinterlacing can be especially tricky if the shot is anything other than properly interlaced 29.97 original. Film transfers with pulldown are tough. Irregular pulldown, common with older film, is really frustrating. PAL transfers (more common than you might think) have all kinds of issues baked in that you simply can't reverse out. Even unusual shutter speeds in NTSC can be an issue.
The go to deinterlace tool of many is After Effects. I find it difficult and slow. I love JES Deinterlacer, especially for off-speed originals, though it is a bit tricky to master. I always attempt the easiest solution first, usually a built-in or add-on. If it looks good, I leave it. If it doesn't then I start analyzing and trying alternatives. It's often best to reverse out any prior speed adjustment first and apply a new speed change. With film chain transfers, you may even want to rotoscope frame by frame before uprezzing. For a ten second shot with a problem spot, this may be worth it, especially if you can tolerate a small speed shift.
Often, you simply need to experiment to get something you are happy with, or you give up and go with what you can get.
