In the Baselight plug in, the "Soften" operator does a very nice job with skin tones.

I had a lead actor with a really bad outbreak of acne and I found Smooth Tone was insufficient to the task, it was a little too nuanced. I used Cartoon Look. I isolated his face using Pixel Chooser and a PC Region and set the PC Matte to Chroma Diff choosing his skintone. I then turned off Lines (the 'inking' part of a cartoon) and adjusted Soften and Cartoon levels until I was satisfied with the result.
I was able to soften his skin as much as I liked (and thought I could get away with) without blurring the background, his eyes, hair, etc. Hardly painless, but it worked.
Apple's Color does (did) a better/easier job of this (I softened the lines on an older actress's face), but it has all it's own issues.
