Yep, an occasional problem with a silly workaround, at least in OS X 10.8.5:
Launch System Preference
Open Displays
Click on Arrangement
Now, re-arrange your monitors, putting one on top of the other until you SEE the missing Avid menu bar.
Drag the avid menu bar/window back to where it should go.
Put your monitors back in proper arrangement
Sigh
On with the show…
