I bought an Intuos 4 a couple of months ago to replace a stolen Intuos 2.
I haven't used a mouse in years, I do everything with the tablet, so when the old one was stolen, I couldn't hold out for more than 2 days before I had to get a replacement.
It works great on both Mac and PC. I haven't tried the multi touch Intuos 5, but I can't see where I'd have much use for it in my daily workflow.
