More testing:

Tried insert edit on pre-striped tape - same error. Tried video only
sequence, same result.

Tried assemble edit with deck set to reference input video - same error.

At this point, I am officially declaring this operation broken. Hopefully
someone at Avid will respond to my trouble reports there.
